How do I Qualify?

1. You must have a permanent disability with documented need for in-home services.

2. You must be Medicaid active.

3. You must be 18 years old or older.

4. You must be able to self direct your own care.

Independent Living Waiver (ILW)

How do I Qualify?

1. You must have a permanent disability with documented need for in-home services.

2. You must be Medicaid active.

3. You must be 18 years old or older.

Non-Medicaid Eligible (NME)

How do I Qualify?

1. You must have a permanent disability with documented need for in-home services.

2. You must be 18 years old or older.
